  1. Complete History
  2. Initial lab test: HbA1C, Lipid Panel (Total Cholesterol, LDL, HDL, Triglycerides), Complete Metabolic Panel, Urine for Micro-albumin/Creatinine ratio.
  3. Initial Co morbidity Screening.
     1. Nephropathy – U/A with Micro/Creatinine ratio/GFR
     2. Neuropathy – Monofilament (annually)/ Sural Nerve conduction study (every 3 years if negative)
     3. Dyslipidemia – Lipid panel (Total Cholesterol, Triglyceride, LDL-c, HDL)
        1. High Risk patients: Carotid ultrasound (Symptomatic)
        2. Ultrasound of the Aorta (Age >65, male with history of smoking)
     4. Vascular complication screening
        1. Ankle Brachial Index (every 3 years if negative)
     5. Ophthalmology – Retinal exams (Annually, Dilated Eye Exam, if needed)

  • A registered nurse is available by phone during Wellness Center hours to assist Patients with health concerns and questions.
